In thefe Words,

A. B. do folemnly and uncerely, in the Presence of God, profels,
teffifie, and declare, That I do believe that in the Sacrament of
the Lord's Supper there is not any Transubstantiation of the Ele-
ments of Bread and Wine into the Body and Blood of Chilt, at,
oz after the Consecration thereof by any Perlon whatsoever; And
that the Invocation o2 Adozation of the Wirgin Mary, or any other
Saint, and the Sacrifice of the Mals, as they are now uled in
the Church of Rome, are Super&itious and Idolatrous.

Published for the Benefit of all others whom it may concern.
